Fitch Ratings released a report indicating that U.S. nonprofit hospital systems have posted rising operating margins for three consecutive years, with the median margin climbing to 1.5% after a historic low of 0.2% in fiscal 2022. The firm attributes recent performance to strong patient volumes, easing labor pressures and the adoption of AI tools. Yet Fitch cautions that the One Big Beautiful Bill Act—legislation signed by Donald Trump on July 4 2025 and also called HR 1—will impose new work, eligibility and certification requirements on Medicaid recipients, cutting roughly $800 billion from the program over ten years.
These cuts are set to take effect in January 2027 and are expected to pressure hospital finances.
