New Study Says Classic Childhood 'Adiposity Rebound' Is Actually Muscle Growth

Research presented at the European Congress on Obesity shows that the long-standing idea of an adiposity rebound in children is likely a misinterpretation of normal muscle and lean-tissue development.

At the recent European Congress on Obesity in Istanbul, researchers led by Andrew Agbaje presented an analysis that challenges the 42-year-old adiposity rebound hypothesis. Using waist-to-height ratio, a more accurate proxy for body fat, they showed that children’s fat levels continue to fall through age seven, while BMI climbs due to muscle and lean-mass growth. The study examined data from the 2021-2023 NHANES cycle, covering 2,410 multiracial participants aged 2-19.

Earlier diet-intervention trials also failed to shift the timing of the BMI rise, supporting the view that the pattern is a normal growth phase, not a disease marker. Agbaje argues that the rebound notion is a statistical artifact and recommends adopting waist-to-height ratio for pediatric obesity screening. A free WHtR calculator has been released to aid clinicians in detecting excess fat in children.
