Council initiates health-and-safety prosecution after toddler dies choking at nursery
Hertsmere Borough Council has launched a health and safety prosecution following the death of two-year-old Sadie Salt, who choked on a sausage at Mini Learners in Radlett.
The prosecution follows Sadie's fatal choking incident on 12 November 2020, when she was eating lunch with 25 other children at the Radlett nursery. She died two days later in hospital. Her parents, Zoe and Adam Salt, praised the council’s investigation and await the inquest for answers. The case revives a 2022 inquest that was previously adjourned. Since the tragedy, the Salt family has campaigned to ban high-risk foods for under-fives in nurseries.
