Federal investigators detail causes of Amazon cargo plane crash at Miami airport

A federal report says a 21 Air cargo Boeing 767 overshot Miami International Airport’s runway, killing five airport workers and injuring five others.

Federal investigators released their initial analysis of the September 6 crash of 21 Air Flight 7598, a Boeing 767-33A carrying Amazon Prime cargo that overshot Runway 30 at Miami International Airport. The aircraft arrived at the runway at roughly 140 mph, remained above the surface, and failed to engage speed brakes or thrust reversers, leading it to collide with a cleaning-crew van and a Toyota Corolla. Five workers—Rolando Aleman Leon, Yoel Rodriguez Narajo, Julio C. Pineda, Carlos Acosta Fajardo, and Javierk Reyes Quevedo—were killed, and five others were seriously injured.

Pilots tried to lift off again seconds before impact, but insufficient runway remained. The Miami-Dade Sheriff’s Office confirmed the fatalities, and the report has revived earlier safety concerns raised by former 21 Air pilots. Amazon has suspended its contract with the carrier, and multiple wrongful-death suits have been filed against 21 Air, Amazon and the flight crew.
