Medicare GLP‑1 Discount Shows Eligibility Limits as Use of Weight‑Loss Drugs Soars

Medicare has started an 18‑month demonstration that offers Wegovy, Zepbound KwikPen and Foundayo for $50 a month, but only to enrollees with Part D who meet a BMI of 35 or higher, or a BMI of 27‑34 with certain conditions such as prediabetes or cardiovascular disease. Patients whose prescription is for a non‑weight‑loss indication, such as severe obstructive sleep apnea, must use regular Part D coverage and face much higher copays.

At the same time, pharmacists report that GLP‑1 drugs are being prescribed to people without obesity who seek rapid cosmetic weight loss, raising safety concerns. National data show a modest dip in obesity prevalence from 2021 to 2023, coinciding with a more than quadruple increase in GLP‑1 prescriptions since 2021, while bariatric surgery volumes fell 34.1% between 2022 and 2024.
