Pentagon mandates testosterone checks for service members aged 30 and older

The Pentagon has issued immediate guidance requiring active-duty and reserve personnel 30 years or older to undergo testosterone screening.

The Department of Defense released detailed instructions on Wednesday that make testosterone deficiency screening mandatory for all active-duty and reserve members aged 30 and above. Men in this age group will automatically have blood drawn to assess hormone levels, whereas younger men will be tested only if a clinician identifies warning signs or upon request. Female service members are not required to have routine testosterone tests; instead, they will be screened for symptoms such as fatigue and irregular menstrual cycles that may indicate hormonal dysregulation, with off-label testosterone prescribed only for certain postmenopausal cases.

The policy, announced by Secretary of War Pete Hegseth last month, is presented as a measure to boost military readiness by addressing energy-availability issues. Medical professionals have voiced concerns that universal screening could lead to unnecessary or harmful treatments, noting limited evidence that such testing improves combat performance. The FDA plans to convene experts to discuss the medical use of testosterone in the near future.
