Rescue Agency Admits Safety Failures After Diver Drowns Saving Girl

A Swedish girl and rescue diver Adrian Willyson Brask died in a capsized boat accident in Lofoten, prompting an internal safety review.

A capsized boat in Lofoten carrying seven occupants led to the fatal drowning of an eleven-year-old Swedish girl and rescue diver Adrian Willyson Brask, who was 36. The agency’s own investigative team identified several safety failures, such as vague and contradictory diving guidelines, lack of proper training for rescue dives, and deficiencies in equipment maintenance and overall safety management. Communication breakdowns on land delayed alerting the emergency guard until the diver was already in distress.

Janne Oddaker, the agency’s secretary general, publicly acknowledged that leadership had not done enough to protect staff and volunteers. In response, the organization has suspended all rescue diving activities permanently, noting that such missions fall under governmental responsibility. The report has been forwarded to the State Accident Commission and the Maritime Authority for further review.
