Twenty years after the landmark paper that earned Shinya Yamanaka a share of the 2012 Nobel Prize, induced pluripotent stem (iPS) cells are moving toward clinical use, with Japan granting conditional approval to therapies for Parkinson's disease and heart failure. Yamanaka, who works at Kyoto University and the Gladstone Institutes, notes that the field now faces a pivotal moment where responsible development and equitable distribution are essential.
Early work showed that only a tiny fraction of iPS cells could become functional heart cells, but advances have improved efficiency and safety, addressing fears that undifferentiated cells might cause tumors. Ongoing trials aim to treat conditions like macular degeneration and diabetes, while researchers explore universal donor iPS lines to cut costs. Yamanaka likens translational research to a marathon, emphasizing that patient benefit must remain the guiding principle.
