Study Links Birth Order to Hundreds of Health Conditions in U.S. Families
A large-scale analysis of 10 million American siblings found that whether a child is first-born or later-born correlates with distinct health risks, identifying 150 conditions linked to birth order.
A joint investigation by the University of Chicago and Columbia University analyzed health-insurance claims spanning twenty years for 10 million sibling pairs across the United States. Across 569 disease categories, the team identified 150 conditions whose prevalence varied significantly with birth order. First-born children exhibited elevated odds of neurodevelopmental and psychiatric diagnoses—including autism, ADHD, Tourette's and OCD—as well as immune-allergic issues such as food allergies.
In contrast, second-borns were more likely to experience migraines, shingles, substance-use disorders and gastrointestinal ailments like gastritis. Published in Nature Health, the research stresses that the findings reflect associations, not direct causality, and acknowledges constraints like the use of commercial insurance data and potential differences in diagnostic practices. Experts offered hypotheses ranging from parental attention patterns to prenatal environment changes. Ultimately, the authors propose that birth order may act as a modest risk marker at the population level rather than a definitive predictor of individual health outcomes.
