The mistake was corrected, pairing Villarreal with Manchester City, but further blunders followed: Manchester United’s ball was left out of the bowl for Atlético de Madrid, and Liverpool’s ball was added instead, resulting in Atlético facing Bayern Munich and United later drawing Paris Saint-Germain.
Atlético demanded an explanation, and UEFA attributed the chaos to a software failure by an external provider. The organization voided the flawed draw and repeated the entire process three hours later, producing a new set of match-ups, with only Lille versus Chelsea remaining unchanged.
