Why Joining a Clinical Trial Remains Tough for Most Patients

Most patients face multiple obstacles—geography, cost, and limited doctor outreach—that hinder enrollment in clinical trials.

Interest in clinical research is high, but practical hurdles keep many patients from enrolling. Over 2,000 adults with chronic conditions indicated willingness to join a trial, while two-thirds reported never receiving trial information from their providers, and only 9% have ever been invited. Recruitment failures affect as many as 86% of studies, and retention poses additional challenges.

Geographic mismatches, travel costs, and out-of-network care further deter participation, prompting the Department of Health and Human Services to consider rule changes and a bipartisan bill to allow sponsors to cover medical and non-medical expenses without tax penalties. Patient-advocacy groups stress the need for clearer communication about available financial assistance and for expanding decentralized trial models.
