Rural doctors thrive amid policy gaps, urging nationwide reforms

Lloyd Nash, a general and acute care physician who has served in Antarctica, the Pacific and remote Indigenous locales, now leads the department of medicine at Albany Hospital in Western Australia. He describes the unique clinical cases and the sense of impact that comes from working at the edge of his expertise. A recent Royal Australasian College of Physicians report reveals that more than 90% of specialists are based in metropolitan centres across Victoria, New South Wales, South Australia and Western Australia, with Queensland at 82%.

This urban concentration stems from training policies that pull doctors back to cities for final years, when personal commitments make relocation difficult. The report urges national workforce planning, plus housing and childcare assistance, to keep doctors in rural areas. Nash co-directs the WA Rural Physician Training Pathway, which places trainees in regional hospitals while providing city rotations and remote supervision, a model gaining popularity. Ongoing shortages contribute to health disparities, with remote populations facing a 1.5-fold higher risk of early death.
